I think that the metaish aspect of the reality vs unreality is drawn straight from the imagery in the episode, where he thinks he sees Victoria everywhere, and the snow when they're together - it's his reality, in some sense. And one of the cleverest aspects of the ep in my opinion is that it's not clear until near the end that Victoria is a very evil thing, so the watcher's sympathy is tugged along.
